Supporting people within the community with social needs

Isolation, bereavement, housing, employment, emotional and mental health and general well-being.

Social Prescribers will build links, and network with organisations in the local community, to ensure that anyone they are supporting, can be signposted to the most appropriate place. 

Connections, Communication, Care and Kindness are key to this role.
