Care Coordinator, Porch Surgery
My role is to support residents who are at risk of being unable to manage at home, for whatever reason, by building a network of care (either professional or social) that supports them at times of increased difficulty or deteriorating health.
I can also be a point of contact at the surgeries, offering advice and guidance and liaising between patients/families, the surgeries, statutory agences and the voluntary sector.
