Carers Together Wiltshire is a partnership of organisations who have come together to provide support, advice and services to unpaid carers in Wiltshire. We carry out the statutory assessments that underpin so much, and offer much-needed practical and emotional support.
This role is based within Alzheimer's Support, Wiltshire's dementia charity.
As Carer Coordinator, you'll be providing tailored one-to-one support, guidance and resources to unpaid carers of people living with dementia in Wiltshire. A key part of the role is carrying out statutory Carers Assessments on behalf of Wiltshire Council, helping to unlock further crucial support.
You'll be networking with other social care professionals across the county, attending meetings and events, including occasionally outside normal working hours. The role is offered on a hybrid basis, with at least two days a week in one of Alzheimer's Support's offices, and travel across Wiltshire.
