The Family Nurse Partnership (FNP) is a preventative program offered to first-time mothers aged 19 and under.
HCRG Care Group provides this program to mothers who reside in Wiltshire.
This program is voluntary and offers 64 intensive and structured home visits. Specially trained nurses deliver these visits from early pregnancy through the child’s 2nd birthday. The visits can occur in the mother’s home or community settings such as a coffee shop, soft play center, or park wherever the mother feels most comfortable.
